Registration Information

Required Documents:
1. Completed registration form
2. Copy of birth certificate or passport
3. School Entrance Health Certificate
4. Copy of Health Insurance Policy
5. Official Transcript / Withdrawal Form
6. Map to residence. Zoning area: Puerto Rico, upper/lower Navy Hill, Garapan, Chinatown, Northern Gualo Rai and Northern Beach Road (Gualo Rai traffic light to Marianas Eye Institute North)

Note:
1. Guardians must provide proof of legal guardianship as notarized and recorded in court.
2. Age of Entrance. Students entering K-5 must be five years old on or before August 31, 2008. Students entering the first grade must be six years of age on or before August 31, 2008.
3. Students will not be officially registered until all the required documents have been submitted.
